The “rights” of medication administration are a safety framework. Different schools list different numbers of rights, but the goal is the same: reduce preventable medication errors.

A route is how medication enters the body: oral, IV, IM, subcutaneous, topical, inhaled, ophthalmic, otic, rectal, or other routes. The route changes onset, risk, technique, and monitoring.

A side effect is an expected unwanted effect. An adverse reaction is a harmful or unintended response. An allergy is an immune reaction and may include rash, swelling, wheezing, or anaphylaxis. A contraindication means a drug or action should be avoided because the risk is too high in that situation.

High-alert medications can cause serious harm if used incorrectly. Examples often include insulin, anticoagulants, opioids, concentrated electrolytes, and certain sedatives. These require careful checks and monitoring.

  • If a patient reports swelling, wheezing, hives, or throat tightness after a drug, think allergy/anaphylaxis concern, not simple side effect.
