Plain-English introduction

Breathing is more than moving air. The body must move air into the lungs, exchange oxygen and carbon dioxide, carry oxygen in the blood, and deliver it to tissues. A problem in any step can become serious.

Respiratory lessons are important for emergency care because breathing changes can be one of the earliest signs that a patient is getting worse.

Airway refers to the path air takes into the lungs. Ventilation means moving air in and out. Oxygenation means getting oxygen into the blood. A person can have a ventilation problem, an oxygenation problem, or both.

Respiratory distress may show as fast breathing, noisy breathing, inability to speak full sentences, accessory muscle use, nasal flaring, retractions, blue or gray color, confusion, anxiety, or low oxygen saturation. The patient’s work of breathing matters, not just the pulse oximeter number.

Asthma involves airway narrowing and inflammation. COPD often involves chronic airflow limitation. Pneumonia involves infection/inflammation in lung tissue. Pulmonary embolism involves a clot blocking blood flow in the lungs. Different causes can produce similar shortness of breath, so assessment and medical evaluation matter.

How to think through questions

  1. Look at the patient before the monitor.
  2. Count breathing rate and observe effort.
  3. Check oxygen saturation if available.
  4. Ask what changed and how fast.
  5. Escalate quickly for severe distress, confusion, cyanosis, or exhaustion.

Transfer and delivery of oxygen in the blood.
Hypoxia
Low oxygen at the tissue level.
Dyspnea
Shortness of breath or difficult breathing.

Bluish color that can suggest poor oxygenation.
Pulse oximetry
Noninvasive estimate of oxygen saturation.

Common mistakes

  • Thinking oxygen saturation is the only breathing clue.
  • Missing fatigue in a patient who was working hard to breathe and is now getting quiet.
  • Confusing asthma, COPD, pneumonia, and pulmonary embolism as one condition.
