Core lesson

Glucose is sugar in the blood. The body uses glucose for energy. Insulin helps move glucose from the bloodstream into cells. When insulin is missing or not working well, blood glucose rises.

Hypoglycemia means low blood glucose. It can cause sweating, shaking, hunger, anxiety, headache, confusion, weakness, and even seizure or unconsciousness. Because the brain depends on glucose, low sugar can become urgent.

Hyperglycemia means high blood glucose. It can cause thirst, frequent urination, blurred vision, fatigue, and slow wound healing. Very high glucose with ketones and acidosis can become diabetic ketoacidosis, especially in insulin deficiency.

A1C is a longer-term marker of average blood glucose over roughly the past few months. It is not the same as a finger-stick glucose, which shows a point-in-time reading.

Sliding scale insulin
Insulin dosing based on current blood glucose according to an ordered scale.

How to think through questions

  • When a diabetes question mentions sweating, shaking, confusion, or sudden weakness, think low sugar first.
  • When insulin is ordered, connect it to meals, blood glucose checks, timing, and hypoglycemia risk.
  • When high glucose appears with vomiting, dehydration, rapid breathing, fruity breath, or ketones, think possible DKA.

Common mistakes

  • Mixing up A1C and current glucose. A1C is longer-term; finger-stick is now.
  • Giving insulin thinking without considering whether the patient will eat.
  • Ignoring foot care and wound risk in diabetes.
  • Treating all high blood sugar as the same level of urgency. Symptoms and ketones matter.
