Chapter 15

Thyroid & Endocrine

TSH, T4, hypothyroid, hyperthyroid, adrenal clues, and hormone feedback loops.

Educational use only: This lesson is for learning and review. It is not medical advice, diagnosis, treatment, emergency instruction, or a substitute for approved training or a licensed professional.

What you need to know

  • Understand thyroid hormone feedback in simple terms.
  • Compare hypothyroid and hyperthyroid patterns.
  • Recognize that endocrine symptoms can overlap with many conditions.

Plain-English introduction

The endocrine system uses hormones as chemical messages. The thyroid helps regulate metabolism, energy, temperature, heart rate, and many body functions. Thyroid symptoms can be subtle or mistaken for other problems.

Lab interpretation should be handled by clinicians, but students can learn what the terms mean. TSH, T4, and T3 are common thyroid words.

Full lesson

TSH is thyroid-stimulating hormone from the pituitary. It tells the thyroid to produce hormone. Free T4 is a major circulating thyroid hormone. T3 is a more active form. In a simple feedback loop, high thyroid hormone tends to lower TSH, while low thyroid hormone tends to raise TSH. Real cases can be more complex.

Hypothyroidism means low thyroid function. It may be associated with fatigue, cold intolerance, constipation, dry skin, weight gain, slow heart rate, depression-like symptoms, or high cholesterol. Hyperthyroidism means excessive thyroid activity and may involve heat intolerance, weight loss, tremor, anxiety, fast heart rate, sweating, and diarrhea.

Endocrine safety includes medication timing, avoiding sudden medication changes without guidance, recognizing severe symptoms, and understanding that other hormone systems such as adrenal, pituitary, and pancreas interact with the body’s stress and energy balance.

How to think through questions

  1. Identify the hormone system.
  2. Compare symptoms to low-function or high-function patterns.
  3. Use labs as clues, not self-diagnosis.
  4. Avoid changing hormone medication without medical guidance.

Key terms to know

Endocrine system
Glands and hormones that regulate body functions.
TSH
Thyroid-stimulating hormone from the pituitary.
Free T4
A circulating thyroid hormone measurement.
T3
A more active thyroid hormone form.
Hypothyroidism
Underactive thyroid function.
Hyperthyroidism
Overactive thyroid function.
Feedback loop
A control system where hormone levels influence signals that increase or decrease production.

Common mistakes

  • Assuming fatigue always means low thyroid.
  • Changing thyroid medication based on symptoms alone.
  • Forgetting that anxiety-like or depression-like symptoms can have medical causes.

Memory hooks

  • TSH tells the thyroid to work.
  • Hypo slows down; hyper speeds up.
  • Hormones use feedback loops.

Mini quiz

  1. What does TSH do?
It signals/stimulates the thyroid to produce hormone.
  2. Name one hypothyroid pattern symptom.
Fatigue, cold intolerance, constipation, dry skin, weight gain, or slow heart rate.
  3. What is a feedback loop?
A control system where hormone levels influence signals to increase or decrease production.

Quick review checklist

  • I can explain TSH and T4 in simple terms.
  • I can compare hypo and hyper thyroid patterns.
  • I know thyroid medication decisions belong with clinicians.
