Plain-English introduction

The musculoskeletal system includes bones, joints, muscles, tendons, ligaments, cartilage, and connective tissue. Rheumatology often focuses on autoimmune and inflammatory conditions affecting joints and tissues.

Pain in this system can be mechanical, inflammatory, traumatic, infectious, or referred from another organ system. That is why location, timing, swelling, stiffness, fever, injury, and function all matter.

Arthritis means joint inflammation, but the causes vary. Osteoarthritis is commonly related to wear and cartilage changes. Rheumatoid arthritis is autoimmune and inflammatory. Gout involves uric acid crystal inflammation. Septic arthritis is infection in a joint and can be urgent.

Costochondritis is inflammation of cartilage where ribs connect near the sternum. It can cause chest wall pain, often worsened by movement or touch. However, chest pain should not be casually dismissed, especially with shortness of breath, sweating, nausea, radiation, or risk factors.

Musculoskeletal assessment looks at pain, swelling, redness, warmth, range of motion, strength, sensation, pulses, deformity, and function. Neurovascular checks are important after injuries or casts because circulation and nerve function can be affected.

How to think through questions

  1. Identify whether pain seems joint, muscle, bone, tendon, or nerve-related.
  2. Look for inflammation signs: redness, heat, swelling, pain, loss of function.
  3. Watch for red flags such as fever, deformity, numbness, poor pulses, or chest symptoms.
  4. Do not dismiss chest pain without proper evaluation.
